Does heat spreaders on performance memories like Corsair XMS, Kingston HyperX really help with overclocking? And does the Twinmos PC3200 without heat spreader work just as good?

I don't think it makes a difference in overclocking. I seen people with Twinmos hit same speed as HyperX and XMS.

In theory, it spreads out the heat for the air to get at it...
And since overclocking usually means higher temps, then yes, in theory it would be better, getting rid of more hot air.

Maybe if the RAM heatspreaders had small fins in them I'd give them a definitive "yes".

My TwinMos don't have any HS on them and they work great.
If they don't come with one I would not put one on them..Don't need it..
