For week nr, check your core, it says:
2100xxyyyyyyy or 2300xxyyyyyyyyyyy with the 21 or 23 for the fabrication site, 00 (or 99) for production year. The next 2 digits are the week nr. your birdy was produced.

For a good idea, check the cpu-database on the homepage, where you have many examples of this nr. and other birds.

On the homepage, there are some articles on AMD coding. You might wanna look there.
