Should i return it to manufacturer and ask for replacement?
Its a vantec 475w stealth psu. I've had it for about 7 or 8 months and it seems to have turned this low recently (not exactly sure when though). Should i rma it?

Did you check with a DMM?
If so, RMA time.

DMM? multimeter..? I dont have one, but in both the bios and mbm they give me the same values 11.22~11.38. The rest of the voltages are fine except for 12v.

Here they are:
Vcore: 1.47
3.3V - 3.31v
+5V – 4.81V
+12V – 11.22V
-12V - -11.88v
-5v - -4.88v

edit: by the way, the computer runs fine. Not OCing right now because of the high ambient temps except for vid card. 9700np at 362.25/660.

The climb in ambient may be to blame, even so it should not be running that low. I would check it with a multimeter while it is under load. If it is fine from that then it may be your mobo's mosfets.

Edit: I didnt notice you didnt have one(multimeter), but you should be able to find a cheap 10 dollar one, it would be accurate enough for this.

Where would i actually measure it from? Any pics or directions that can help me out?

You would measure it from one of your power connectors that hooks to a drive, touch the positive of the multimeter to the the little prong in the connector that goes to the yellow wire(it should be on one of the far sides) and the ground of the multimeter to the prong with the black ground wire.(should be right next to the yellow one) Try and do this while folding or using prime95 or something that will put it under load. If it is less than 11.7v then RMA it, if not then it is probably your mobos fault.

both the bios and mbm they give me the same values 11.22~11.38.

But both rely on the mobo hardware, which isn't known for high accuracy. I had one measurement disagree with my 0.4%, +- 2-3 count voltage meter by about 6%, and another person measured a discrepancy of 6.99%.
