For the pop3/smtp. call them and ask them, they will tell you ;)
You could always send an email to yourself and view header information (if your lucky) and pull the servers from that. There's a couple other ways but try calling them first.
And is this Outlook Express? if it is, I dont think you can get your address book back since Outlook Express is a part of windows and therefore over-wrote everything. Happened to me on my last "repair" which ironically broke more things than it fixed...but thats another thing altogether

servers for comcast = mail.comcast.net and smtp.comcast.net
I'm not sure if you can get back ur address book but for future reference always export mail and personal folder from outlook before repairing or reloading.

do you know where you SAVED address book in your computer ???
if you can't find address book in your computer, tell your friends/ relatives to email you again. When they do that, you will have your address book back because outlook will automatically add them in address book if you allow it to do that :)
make sure you back up it for future reference

If you're referring to OE, then the following article may be of some help to you...
The Windows Address Book (WAB) (http://www.tomsterdam.com/insideoe/files/wab.htm) :)
