What is with my 12v RAIL?

Why is my 12v rail @ 13.6 when full loading @ 1040 2.2v? Why is it idleing @ 13.2?

I know for a fact my 250watt psu can't handle anymore, but why is this happening?

Will i get a better overclock when i change to a 400watt antec?

that happens to me, my 5v goes down and my 12v up.

You shold most defintly upgradeyour psu and yes you probably will overclock higher.

The 12 volt rail doesn't have anything to do with the core voltage, the 5 volt rail does. As for upgrading it, there are no guarantees. You'll probably want to just to meet AMD's recommendaations (unless you already have)
