If the case fan is 120mm. Is the 120mm the diameter of the circle containing the fins or is it the whole square case fan?

It's 120mm square, in other words it's 120mm from left to right and 120mm from top to bottom. The body housing the fan is 120mm.

so what size hole should I cut in the case if the fan is 120mm?
A 120mm diameter circle. That will leave all of the fan's blades exposed to move air but you'll still have area to attach the fan in the corners.

Just eyeball it. Either pick a bit big enough to let you put a screw through the metal, then into the plastic of the fan and have the screw be big enough to dig into the fan housing to secure it that way or use a nut and bolt setup, that way it doesn't matter if your drill holes are a bit too big as long as you use washers and really tighten things down good.
