Pump, radiator w/fan, waterblock, tubing
www.dangerden.com
www.overclock-watercool.com
Get a waterblock from dangerden like the maze2, you can also get an ehiem pump there. overclock-watercool.com carries the black ice radiator which has tested very well, if you can order from overclock-watercool's sister site (it should be listed on overclock-watercool's site) because OC-watercool has a bad reputation with getting orders out.
Oh yeah- www.2cooltek.com for a 120mm fan (I suggest the YS-tech)

i agree with that, only thing is you might want to run the ys-tech fans at a slower speed, possably 7 volts because of the noise of the air

I just did a little article with alot of good links and suggestions on parts for watercooling. Won't help you much with picking a case and sizing like you asked for, but the links are handy.
Right here (http://www.procooling.com/articles/html/unaclockers_h2o_buyers_guide_-.shtml)
una, that article is right on ;)
