I have run into this before, but never could fix it. I have a high res monitor, 1600 x 1200. V-sync does not stick, even when enabled in catalyst, even when enabled in game. I always get little horizontal lines while playing. The same thign used to happen all the time on my 8800GT.

Not sure what do do, It simply RUINS games. I get all torn out of game's world/reality.

Latest drivers and a clean vista install here. Proc is an e8400 @ 3.6.

What do you mean by little horizontal lines?

No vsync results in tearing... top half and bottom half of the tear are different frames.
PSU noise or a bad earth loop or current going into an earth results in ripple.. little waves traveling vertically up the screen.

You could try a tool included in rivatuner called D3DOverrider to force vsync and triple buffering.
