ok well my friends system is running unstable and am trying to figure out why.
it is a non-overclocked system with general stuff its a t-bird.
his +5v = +4.56
so i was thinking it might be his PSU what do u guys think?

Wihout a doubt that PSU needs to go. Keep in mind this is only one of many variables that will contribute to an unstable system. 4.56 is rediculously low and borderline dangerous to the rest of the components.
At that level I'm surprised the machine will "kick over" on startup. Thats hideously low. My system won't tolerate 4.79v on the 5v line. I'd lose that psu in favor of at least a 300w one. What guts does the system have within? How many drives, etc.

My 5+ is running at 4.78 which is diabolical but has gone down since i oc the cpu.
its a 300 watt psu but i got it for £16 so that cant be bad but its going to what is a good number to have in +5 category!
And im after a quiet PSU thats good any ideas on where to go ??

i have an antec400 +5 = 4.95 - 5.02
seems quiet
(dunno damn delta 38 cfm)

Over 5V is good, or no lower than 4.85.
check it with a multimeter before condeming the PSU
