video problem

any ideas what could be causing this?
It happens in other games also, this is just the most recent that i've taken a SS of

heat or a big bad overclock lol...Or the card is dieing

I did overclock it once about 2 years ago, used rivatuner i think but from what i've read doesn't that just change registry settings or something? or is there a hardware thing i need to do to undo that? I have reformatted several times since then.
