Help convert a form to HTML

If you have no HTML skillz the easiest way to do it would be to do a convert to HTML function in MS WORD. You could maybe use frontpage too, but if you want to put this online or store info from it into a database, your best bet is to go to www.w3schools.com and start in on their HTML tutorials.
 
But just simply having a form does not mean people will be able to submit information to you. You need a script for the form to submit TO, and have that script do whatever's appropriate with that information (store it in a database, send you an E-Mail, whatever).
