Between 2009-2012 I bought one each year but typically every 2 some odd years otherwise, went from an old FX5700 or X1950Pro to HD5770 to GTX560Ti to HD7850 in the past 3 years.
 
My actual upgrade history m shows I'm not a big fan of "inbetweeners"

I went from a 6800GT to a 9800GTX (This wait hurt), then I got my first 480GTX. I upgraded to a 680 when I started working with multiple computers, then filled in the gaps with used cards as they became available :D

There was a couple "inbetweeners" along the way, mainly for secondary rigs and the sort. among these was an ATI 9100, a HD 4525 (I think), and a 6750 that I sold, then replaced with my old 480 and one I got from a guy for next to nothing, viola her SLI setup
 
Between 1999-2007 i easily bought 1-2 new video cards/year. But since then, cards seem to last much longer, in 2008 i bought a 4870, used it for 2.5 years and in late 2010 bought a gtx 570 that i'm still using now just over 2 years later. Have no reason to upgrade yet.
 
Once a year.Its nice have something new that will handle the newer releases better, but now I have video cards laying around that I don't use.I'm not really a seller when it comes to pc parts.I justify this by saying, I will put it to use eventually.
 
I use a single card on a single 24" IPS monitor. I get a new card when the old one breaks or no longer plays my games. I try to get the best card I can afford so I can swap it out less often. My GTX 285 turns 3 years old this summer but am trying to hold out for a GTX 780 Hydrocopper in 2014. If some new game comes out I must play in 2013, will go with a GTX 680...or a Titan. Can't wait. :)
 
In the last 10years thAT I remember, I went from some 32MB geforce type, to

geforce 2 MMX440 64MB
ATI x1350pro 256MB
5670 1GB
6950 2GB then crossfire
7950 3GB now crossfired

Depending on how they do with 2560x1600, I should keep them until mid 2014, but I like to sell them before they break or loss most of their value

Way before all that it was commordore64, then something in a 486DX2 66MHz, then something in a "pentium" 100MHz
