Strange read performance from Agility 3

I purchased an Agility 3 new back when it was still a new/up to date product(so ive had it for 3-4 years now?). Ive been noticing strange behavior when it comes to game loads(its a gaming rig only), where loads will vary immensely and in an open world environment(Skyrim ect) i get the jitters as i cross regions. Load screens that used to flash and be gone instantaneously are now taking up to 10 seconds to load, and even loads that happen quickly take a few.
Ive re-installed windows, and there have been countless install/deletes on the drive(i only keep windows and 1-2 games max on the drive, then delete and install the next game).

For giggles I ran an HDTune test and the read came out very strange

Is this the result of wear out? Is the drive done for?
 
I fail to understand why one would uninstall a game only to install another. Keeping 1-2 games on the drive is fine, but all that uninstalling/installing wears out SSD's, and my money is on the drive wearing out because of that :(

Yea, i figured as much, its only a 128GB hard drive, and with the size of games today anymore then 3-4 will use up more then 80% of the drive
