only if it causes crashing or instability. I've seen plenty of PCs give a garbage reading on the 3,3V line, due to a crappy HW monitor IC. I just don't trust them anymore. The +5V and +12V readings are the only ones I (usually) believe.

If the system is unstable, I'd replace the PSU. But if it runs fine, I'd leave it be.
 
In many cases you would notice that if the voltage is fluctuating it could be nothing..

In many of those cases if you grab a multimeter and measure it you would notice that the voltage is in fact not changing at all..
