You don't have to; mine is still on and I use an old GF2/3 waterblock on it (flat base no indents for shim) and it works fine. I did however use a little extra Arctic Ceramique to make sure there is contact. You could always file down/dremel the edges of your old heatsink so it fits in the shim area properly.
 
Apparently the Shim doesn't provide the heatsink with very good contact to the GPU - I'd take it off and just be careful about how much pressure you mount the sink with.

To get it off, you should be able to use the freezer trick with an exacto knife to pry it off there.
