front audio ports

OK i need audio ports on the front of my pc(for headphones). I'm looking to do it myself and dont want a pre-maid controller. i have a soundblaster live value with digital out front and rear out. I have a y-split on the audio out to go to my home amp and one to my computer speakers (2.1). problem having to pull the computer out and hook and unhook when ever i need the headphones.

as stated I have a soundblaster live value (with digital out port)if that matters
a qdi avdance 10t MB has onboard sound(disabled)

can I run fromt ports with out losing the rear connections..
