Ati (Sapphire) Black PCB 9700 PRO 128MB w/ ati silencer 1 installed
Newest Omegas (I have tried 4.10 same problem used driver cleaner 3 in-between them)
I am not OCing anything on my system right now. The card has been as high as 371/318 (safe according to atitool) and downclocked and the problem remains EXACTLY the same.

Sapphire Radeon 9700 Pro 128mb
3700AMB Antec Case/420 watt thermaltake silentpower power supply
 
yup, looks like that card is overheating, althought its not as bad as it could be. all i can suggest is that you check if its installed correctly (the silencer)
 
I had a weird artifact problem like this on an old Geforce 3 after changing heatsinks. I would refurb it if you still can, it might be that the memory is fuxored
