Help convert a form to HTML


Can anyone help me convert a regular print and mail-in registration form into HTML.

If you have no HTML skillz the easiest way to do it would be to do a convert to HTML function in MS WORD. You could maybe use frontpage too, but if you want to put this online or store info from it into a database, your best bet is to go to www.w3schools.com and start in on their HTML tutorials.

Its not really that great. Its quick and dirty but it does the job.

But just simply having a form does not mean people will be able to submit information to you. You need a script for the form to submit TO, and have that script do whatever's appropriate with that information (store it in a database, send you an E-Mail, whatever).
