I called MGE and talked with a guy named Umar.

He said to send UPC with Rebate number 11112 and Serial with rebate number 11113.

From another thread I believe "Umar" is Umar Khan and people who have had issues specifically mentioned Umar Khan at MGE. If you do this be prepared for a fight.

In addition, searches on MGE rebates are all negative. Evreyone had to hassle them (apparently "him", since evreyone got the same guy) for months to get anything back. No go.
