The reviews i've read about the various coolers built like that have said that they don't work very well.

usually mounting vertically, as in the mobo is horizontal, will provide slightly better temps if the heatsink is heavy enough

Actually, tests show that when the motherboard is perpendicular to the axis of the floor (as in a typical tower case setup) temps are slightly lower.

Orientation of the cooler with regard to exhaust fans does matter, however, especially when you are using a tower-style cooler whose fans blow air parallel to the motherboard. An exhaust case fan located downstream from the air passing through the cooler is most efficient.
