Don't know if you've tried it this way, but some USB devices are little bastards.

Make sure you removed all drivers the USB device you're wanting to install. Reboot. Reinstall the drivers for your device. Shut down the PC. Plug in your USB device. Boot back up. It should detect the device and automatically install the driver from the driver cache.

Let me know how it turns out.

on the EpoX 8K7A/+ slot 1 and AGP shared, Slot 2 shared with raid or not shared if no raid, slots 3-5 are shared together with onboard audio, and slots 4-6 are shared together with USB.

Try to avoid sharing this may help.

So are you saying I shouldnt put any pci device in slot 4 and 6??

8k7a USB issues...AND a Solution....

I just spent a frustrating weekend trying to solve the same problem. I dropped a Tbird 1.33@266 on a 8K7A+, thought that all was well - then I loaded my stick and keyspan remote. Same thing you describe - It would initially install but on the reboot, all I would get was "unknown USB device". I thought I tried everything - different drivers, different PCI slots, etc. I finally cleared the CMOS and started from scratch. Proc showed up as a 900 (9x100). WHAMO! everything showed back up. OK, lets try 10x100. Everything still showed up. The problem I had was that the USB devices wouldn't show up at any FSB over 104!!! Even if I underclocked the proc (6x133), they wouldn't take a FSB of 133. To make a long story short, I kicked the voltage up a notch (1 step up from default) and that has solved the problem. I finally solved it and I have been rock solid ever since. Let me know how you make out...Good Luck!

Specs:
8K7A+
Tbird 1.33@1533 (haven't even started to push it yet)
Antec SX635
512 MB Crucial PC2100
Millenium Glaciator
MSI 64MB Geforce2 Pro
Phillips Acoustic Edge
IBM 30GIG ATA100 7200rpm
Teac 12x10x32
WIN98 SE
etc...etc...etc...

I use a usb logitech optical mouse and havn't had any trouble with it all i did was enable usb mouse support in the bios and everything is dandy.

Changing AGP drive strenght to EE in bios solved any USB problems for me. (Asus A7V)

The thing I've noticed is that some genius decided to share the USB and AGP voltage. If you want more or less trouble free operation of USB devices; pick up a cheap self-powered USB hub.
