fanbus, again

I'd like someone to wrte out step by step how to create a 4 switch fanbus w/ dual color ultra-bright LEDs (if they exsist) with green being high and blue low if I can get it like that. I've seen he ms paint diagrams and dont understand them very well.

I'd like to put all this on a circiut board if anyone could show me how.

I would buy the pc-mods one but it wont fit where i want it to go..
