Something's very wrong with that HWinfo, too many temps too high. CPU isn't supposed to go near 70c (optimally kept below 62c) let alone 90c. Either that or your fans aren't working and you might experience a fire in the near future...

What do you have on there for a cooler .... even the stock cooler from AMD should be adequate at stock. You have a cooling issue.

Here you go^, as well as the fact that your VRMs are probably hot enough to grill fish on. Check thermal paste and cooler install, case airflow, and then lock the frequency at 4 GHz and start walking back the vcore as far it will go and stay stable. Then turn on all the green stuff.

edit: This in addition to a fan on the socket and one on the VRMs. Best option would be file that motherboard under 'bad idea' and look for an good Asus (#1) if overclocking or Gigabyte(#2).
