Need settings for MS 6905 Master Slocket!

Hi, I was hoping someone could help me with this. I own an MS 6905 Master Slocket adapter (revision 2.0 I believe). When I bought this slocket it came with no documentation of any kind. All of the settings were recorded on the back of the box. At the time I cut the back off the box and saved it...

Unfortunately I seem to have misplaced the settings diagram! I checked the MSi Site for the slocket settings, but they have no information on the slocket at all (it's almost as if they want to pretend it doesn't exist!). Without these settings the adapter is pretty much useless to me, since I can't set it up properly. I would greatly appreciate it if someone could post the Slocket settings here, or link me to a site with the settings! If you could scan the back of the Slocket box which has the settings that would be great!
