visiontek 6564 UPC question

This might be a dumb question, but does anyone know if the Visiontek 6564 retail box has the same UPC now that it did back in November? The reason I ask is I bought the card from Best Buy after Thanksgiving when they had the two $50 rebates. I sent my rebate form in to get the mail-in rebate, but it got lost. I forgot to make a copy of the UPC I cut out of the box. I'm currently trying to figure out how I can get a copy so I can resubmit my rebate form(I think they might let me do that).

It would really help me out if someone else who got the Thanksgiving sale rebate and made a copy of the UPC would let me know what the UPC number is.

hawkeye, what do you mean it got lost? I sent mine in about 2 months ago and haven't heard anything. Starting to get worried.

JAWS, the rebate took a long time to arrive. I sent mine in the third week of December and just got it last week. You can check your status on wheresmyrebate.com or call them. My status didn't show anything but they had my name wrong in the database.

jaws, I mailed my rebate form at the end of December. When I still had not received a check by last week, I called Visiontek. They told me they have no record of any rebate form sent in by me. Either it was lost in the mail or Visiontek lost it.

Damn, looks like Best Buy is not going to give me another rebate form from last Thanksgiving. They still have it in their system, but they won't print one out for me.
