Does anyone know what might cause this error on gpu? It has happened twice.

Just a FYI Hardass, I have received that error when using the wrong driver for my GPU. Albeit it was on my Radeon 5850 so it may have been because it's an older Gpu.
 
I am running latest drivers, the system has 2x 7970s in it but only one gets the error. I have put cards back to stock to see if a overclock is the culprit.
 
Heat most likely. I had the same issue with a 7970 and 280x. Top card was ok but the lower card was getting to hot.

Cards are watercooled. But they are not a matched pair. Same mfg but one is a OC version. No errors since set back to 1030 core.:)
