Need some kind of script to add text to image file

Here's my situation. I take a picture of a cardboard box. Now let's say this particular box is named Jim Smith. It has a number on the side of the box, and a number on top of the box. Both of those numbers are specific to Jim Smith.

What I'd like, is to be able to open the picture of the box, and get prompted for First Name (Jim), Last Name (Smith), number (side of box), number (top of box).

Any ideas?

I'm not really taking pictures of cardboard boxes.
