Lately I've had some problems with my powercolor radeon 6850 card.
The problem occurs now and then (lately more often than before) , most often during gaming (bf2 , bf3 etc) but occasionally just surfing web /casual computer use etc.

The screen suddenly gets mirrored.
I didnt get the chance to take a picture with a camera but heres my rendition of what happens in photoshop:
http://imageshack.us/photo/my-images/259/artifactsc.jpg/

Basically the screen splits in 2 and the left moves to the side while the right shows whats on the left .The computer continues to work and there are no BSOD's but I have to restart to get the screen propper.
The display pattern is always the same until yesterday i got a crash with just grey/black lines or dots all over the screen (that time it crashed).
I got a crashdump in the event viewer pointing to atikmpag.sys
I uninstalled the display driver and installed the latest drivers but after playing about an hour of bf3 it crashed once more (with the image mirroring again).

In the past I had bsod's involving atikmpag.sys
but they stopped after I disabled hardware acceleration in flash for 2+ months.
How likely is it that my GPU is failing?

Here are some crashdump information :

On Fri 5/25/2012 7:39:40 PM GMT your computer crashed
crash dump file: C:\Windows\Minidump\052512-10327-01.dmp
This was probably caused by the following module: hal.dll (hal+0x12818)
Bugcheck code: 0x9C (0x0, 0xFFFFF8800336DC70, 0x0, 0x0)
Error: MACHINE_CHECK_EXCEPTION
file path: C:\Windows\system32\hal.dll
product: Microsoft® Windows® Operating System
company: Microsoft Corporation
description: Hardware Abstraction Layer DLL
Bug check description: This bug check indicates that a fatal machine check exception has occurred.
The crash took place in a standard Microsoft module. Your system configuration may be incorrect. Possibly this problem is caused by another driver on your system which cannot be identified at this time.

On Fri 5/25/2012 7:39:40 PM GMT your computer crashed
crash dump file: C:\Windows\memory.dmp
This was probably caused by the following module: atikmdag.sys (atikmdag+0x3BA09)
Bugcheck code: 0x9C (0x0, 0xFFFFF8800336DC70, 0x0, 0x0)
Error: MACHINE_CHECK_EXCEPTION
file path: C:\Windows\system32\drivers\atikmdag.sys
product: ATI Radeon Family
company: Advanced Micro Devices, Inc.
description: ATI Radeon Kernel Mode Driver
Bug check description: This bug check indicates that a fatal machine check exception has occurred.
A third party driver was identified as the probable root cause of this system error. It is suggested you look for an update for the following driver: atikmdag.sys (ATI Radeon Kernel Mode Driver, Advanced Micro Devices, Inc.).
Google query: atikmdag.sys Advanced Micro Devices, Inc. MACHINE_CHECK_EXCEPTION

No longer use the 6850 dude..
If it cannot work at stock speeds under stock voltage then its a faulty card.
I use a Asus directcu2 GTX 670 now no more problems.
