I'm in Jail!

Darn it eh. It still works just looks like crap, couple resets fixes it. But it is very annoying.

The model of Videocard:
ATI Radeon 9500 pro /flashed with the warp bios
Fan: Using Arctic Cooler Vga Silencer
How Long : I had this card since last christmas.
Overclocking : I did partake in overclocking it,but after reinstall awhile back I just left the thing at stock speeds.

It's not overheating,since
A) I can play games for hours and nothing freezes etc
B) It has an arctic cooler on it
C) It does this from a cold boot
D) It's not overclocked

The powersupply I currently have hasn't given me any problems with this card before.
My current bios ps volts readings are
12.0 - 11.97
3.3v - 3.32
+5.0v - 5.04
Bat - 3.16
+5vsb - 5.10
I have no idea,I just need some suggestions on what possible the problem could be?

I just pull the card out and tried it in another system,it worked fine. Then again after I put it back in my system it's working also. After I turn my computer off overnight it will once appear again like clockwork.

Hey this is what happened while playing DOD last night.
It never froze and went away after couple minutes.
