AMD 8150 FX OC - can't get it stable

Hallo guys, I'm fairly new to hi-end computer setup managing, but recently I have both new PC and want to try it for some OC. Here is my PC spec:

CPU: AMD 8150 FX Black Edition with stock water cooler;
GPU: MSI GeForce GTX660, 2GB, GDDR5, OC
MotherBoard: Asrock 970 EXTREME3
PSU: Aerocool VP-750, 750W, aPFC


I have played around ADM OverDrive application, but haven’t received any “good” result. I have read that these CPU’s can handle 4.5+ Ghz OC, but my max was 4.2. After 4.2 my PC just freeze or sometimes even do not boot up. I have turned off Turbo boost – no luck :( . Am I missing something? Is there someone who has same or similar setup? – How far have you pushed it? – What’s your configurations and what I’m doing wrong? :cry:
is it even worth it, to OC or OC after 4.2Ghz?

And this is screen capture of HWMonitor (free version) from CPUID com
HWMonitor has been scrolled enough and large enough to show Min/Max of Voltages and includes the CPU CORE TEMPS fully visible.

This capture is made of HWMonitor after it has been open on the desktop logging Min/Max temps and voltages while Prime 95 was running Blend Mode test on all cores for at least 20 mins and then the capture of HWMonitor was made and it shows the Min/Max temps and voltages before P95 Blend was started and while running P95 Blend mode and gives much greater insight into how the system is performing without guessing.

In order to attach screenshots of images as suggested, first crop and capture the images with Snipping Tool found in Windows Accessories or equivalent. Then click on Go Advanced, a button at the bottom of every new post window. Then click on the little paperclip tool at the top of the Advanced post window when it opens. Clicking on the paperclip tool brings up the file browser/upload tool and the rest is fairly obvious.

did you disable ALL the power saving options in the bios? what are you using to overclock? the multiplier or fsb? if you are using the multiplier then it simply a matter of you not having enough voltage. Can you show us a screen shot?
 
I did use multiplier ( my maximum stable was x21), atm i'm at work, but i will provide with all screens of cpu-z and AMD OverDrive. I have not set any other options than - voltage, multipier ( no brige voltage, no fsb). By default i use CPU votage on ~1.36XXX but i tried with 1.4XXX and 1.5XXX no luck
