If you're really stuck, consider taking loosing the plastic "roof" of the case and mounting your floppy, optical drives or res up there. If ugly is a problem, you can then make an attractive custom part to replace the lid you ditched.

Floor mounting worked perfectly well for me.
I installed a sub-floor (1/8" black plexi) spaced 1/2" above the real floor and mounted the rad to it.
I did not enlarge the stock floor vent although I did remove the grill.
Used the CM supplied casters.

Running a Weapon 342 single pass rad/shroud with two 120x38mm YateLoons @7v.

If you flip to pseudo-BTX, this config also has the added benefit of pushing a lot of air over the RAM and to a certain extent, the PWM area (depending on your motherboard, of course).
